Eligibility

To be eligible for our services, an individual must be a resident of Northern Indiana and the surrounding cities, and be receiving or about to receive cancer treatment as defined below:

• Radiation Therapy
• Chemotherapy
• Mastectomy (Post-Operative)
• Extensive Surgical Procedure
• Biologic Therapy for Metastatic Cancer

Exclusions for eligibility: Oral therapy (e.g. Tamoxifen) or lumpectomy as single treatment.
